Visit and choose your suite.

Typical timing: day 1.

Complete the application.
One form for the resident and one emergency contact. Download below.
Typical timing: within a week of the tour.

Physician's statement.
Download the form, book an appointment with the family doctor, and bring it back to our Director of Care. This is usually the slowest step, so start it first.
Typical timing: 1–2 weeks.

Nursing assessment and care plan.
Our Director of Care meets the resident and family, reviews the physician's statement, and recommends a care package (select 4, 7 or 11+).
Typical timing: 1 hour, within days of step 3.

Review and sign the residency agreement.
We walk through the information package, the monthly rate, the care package and the move-in date. [Payment at signing: to be confirmed]
Typical timing: 30 minutes.

Prepare the suite.
Measure furniture against the floor plan, label clothing, transfer prescriptions to [pharmacy policy: to be confirmed], arrange mail forwarding and a phone or cable line, and give us copies of power of attorney documents.
Typical timing: the week before.

Move-in day.
Arrive at [to be confirmed]; we book the elevator, meet you at the door, and the welcome meeting covers meal times, the call system and the activity calendar.
Typical timing: half a day.

The first 30 days.
A care plan review at week two, a family check-in at week four.

Completed medical forms contain personal health information. Please return them by fax to (905) 358-2502, or bring them to the residence. Do not send them through this website or by email.
